People who are narcissistic have very little self-awareness. They have very little, what we call, self reflective capacity. They don’t have that ability to go inward and think about how they affect other people, which is a core part of empathy. So when it comes down to it, you have to swallow the bitter pill which is: They actually really don’t care. But the worst part is, they think they do.
